Country Manor Pet Care Service Agreement 

This agreement is made between Country Manor Pet Care and the pet owner(s) listed below. The following terms outline the services provided by Country Manor Pet Care, which include in-home pet sitting and other related pet care services performed at the client’s residence.

Please note: We no longer offer doggie daycare or boarding services. All care is provided exclusively in your home to maintain your pet’s comfort and routine in their familiar environment.

By signing below, the owner(s) confirm they have read, understood, and agreed to the terms and conditions outlined in this agreement.

Participation and Risk Acknowledgment

The owner(s) understand and accept that activities their pet(s) may participate in—such as walks, playtime, or general care—may carry inherent risks, even when precautions are taken. These risks may include, but are not limited to:

Exposure to parasites, viruses, or illnesses that can be transmitted between animals or humans

Minor injuries such as scratches, fatigue, or stress-related behaviors

More serious issues such as sprains, allergic reactions, or in rare cases, fatal outcomes

Country Manor Pet Care takes every reasonable step to ensure a safe and healthy experience for your pets. However, the owner(s) acknowledge that unforeseen incidents may occur and agree that the benefits of in-home care outweigh the potential risks.

By signing this agreement, the owner(s) voluntarily release and hold harmless Country Manor Pet Care, its staff, agents, and representatives from any and all claims, liabilities, or demands arising from the care of their pet(s), including those resulting from the care provider’s own negligence.

Authorization for Emergency Veterinary Care

Should the dog(s) become sick or injured while in the care of Country Manor Pet Care, we will make every reasonable effort to contact the owner(s) using the information provided. If the owner(s) cannot be reached, they authorize Country Manor Pet Care to seek veterinary attention deemed necessary. The owner(s) agree to assume full responsibility for all costs associated with the medical care provided. In the unfortunate event of the dog(s)’ passing, the owner(s) or the emergency contact will be informed immediately.

Health and Vaccination Requirements

The owner(s) attest that, to the best of their knowledge, their dog(s) is free from contagious illnesses and has not been exposed to any communicable diseases within the last 30 days. By bringing their dog(s) to Country Manor Pet Care, the owner(s) affirm that their pet(s) is in good health and up-to-date on all required vaccinations, which MUST include Rabies, Distemper & Bordetalla.

Country Manor Pet Care reserves the right to deny entry if vaccination records are expired or incomplete.
The owner(s) are responsible for providing updated vaccination records via email, text, or hard copy as needed.

Dietary Needs, Allergies, and Medications

The owner(s) agree to disclose any allergies, special dietary requirements, or medications their dog(s) may need during their stay at Country Manor Pet Care. All necessary instructions for care must be provided in advance to ensure the safety and well-being of the dog(s).

Behavior and Temperament

The owner(s) confirm that their dog(s) does not have a history of aggressive behavior, including biting or attacking people or other animals. Country Manor Pet Care reserves the right to refuse services if a dog displays unsafe behavior.

Use of Photos and Media

The owner(s) consent to Country Manor Pet Care using photographs or videos of their dog(s) for promotional purposes, including but not limited to social media platforms, the company website, and printed materials.

Payment Terms

Payments for services must be made in full before the dog(s) is picked up unless alternate arrangements are agreed upon in advance.
Country Manor Pet Care accepts cash and credit card payments.
The owner(s) are also responsible for any additional costs for services rendered during the dog(s)’ stay.
